PSEO Notice of Student Registration
The PSEO Notice of Student Registration (NOSR) form (pdf) is a state required form. Students must complete section one with a parent (if under 18) then have their homeschool or high school complete section two. The PSEO NOSR form allows for typed signatures and can be completed electronically.
 
High School Transcripts
High school or homeschool transcripts can be unofficial or official. If you are homeschooled or your school does not use class rank/GPA, or you do not qualify with your class rank/GPA, you will also need to submit a copy of a nationally standardized test score. 
 
Students applying for the 10th grade Career and Technical option must also include an 8th grade MCA reading score, either on a report or as part of the high school transcript. Homeschooled 10th grade applicants, or high school applicants who did not take the 8th grade MCA reading test, should submit their NOSR form and transcript and indicate that they have not taken the 8th grade MCA. The PSEO team will follow up with details on how to complete the alternate eligibility assessment.
 
An immunization form will also be required of homeschooled students by the 45th day of classes in their first semester.

We are unable to take photographs of documentation but can accept clearly scanned materials and electronic PDFs of transcripts and PSEO Notice of Student Registration forms.

Please allow two weeks for your application to be processed. All applications are processed at the Cambridge Campus for both Coon Rapids and Cambridge Campus students.
